Use this form to ask for a copy of the personal data we hold about you, to correct something that is wrong, or to have your data deleted.

We verify who you are before we act, because handing health information to the wrong person is the worst possible outcome. That usually means confirming the email address on the account.

We respond within 30 days. If a request would delete records another person's care depends on, or that we are legally required to keep, we will explain exactly what we can and cannot remove.
